Abstract
Multiple sclerosis (MS) is a neurological disease that typically affects young women of reproductive age. There are still many doubts and heterogeneous clinical approaches partly due to the lack of consensus and guidelines. For many years, women with MS have been discouraged from pregnancy for fear that the disease will negatively affect the fetus or increase their obstetric risk or because the disease could be related to reduced fertility. However, fertility does not seem to be impaired to a larger extent in women with MS. Therefore, all healthcare providers involved in the follow-up of multiple sclerosis patients must be prepared to discuss future fertility, pregnancy and others issues and provide the best possible counselling. Thus, the present study aims to collect data based on updated evidence to discuss fertility and pregnancy in patients with MS with respect to the impact of pregnancy on MS risk and prognosis, the impact of MS on pregnancy outcomes and in treatments used to restore fertility in these women. In conclusion the relationship between infertility and MS is not yet well established. It seems to be a connection between aggressiveness and disease progression with several processes that can impair fertility. However, MS does not contraindicate the use of assisted reproduction techniques as needed. From the several studies analyzed, it is possible to conclude that pregnancy is feasible in women with MS. It is important to discuss and plan the ideal time point of treatment, pregnancy and contraception aiming at better results.
