Case report - Spontaneous pregnancy after application of PRP in ovaries of patients with premature ovarian failure

Abstract

Premature ovarian insufficiency is characterized by loss of ovarian function before the age of 40 and is associated with infertility, hypoestrogenic symptoms, elevated gonadotropins, and markedly reduced ovarian reserve. In women wishing to conceive, oocyte donation is commonly considered the most effective therapeutic option. However, regenerative approaches, including intraovarian autologous platelet-rich plasma (PRP), have been proposed as potential complementary strategies to improve ovarian activity. This case report describes two 35-year-old women diagnosed with premature ovarian insufficiency who underwent intraovarian PRP administration. Both patients had very low anti-Müllerian hormone levels, elevated gonadotropins, absent or extremely reduced antral follicle counts, infertility, and symptoms compatible with ovarian insufficiency. Autologous PRP was prepared from peripheral blood and injected into both ovaries under transvaginal ultrasound guidance, with repeated application according to follicular response. After treatment, both patients showed follicular development and subsequently achieved spontaneous pregnancy without in vitro fertilization or oocyte donation. In the first case, pregnancy progressed to term, resulting in the birth of a healthy girl weighing 4,035 g. In the second case, early pregnancy was confirmed by rising ?-hCG levels and ultrasound evidence of fetal cardiac activity, and the pregnancy also progressed to term with the birth of a girl. These cases suggest that intraovarian PRP may be associated with restoration of ovarian activity in selected patients with premature ovarian insufficiency. Nevertheless, evidence remains limited, and controlled studies with standardized PRP preparation and application protocols are required to determine efficacy, safety, mechanisms of action, and appropriate patient selection.
