Abstract

OBJECTIVE: Poor ovarian response is always a problem to an IVF center. GnRH antagonists cycles with milder ovarian stimulation (aromatase inhibitor plus gonadotropins) could be the answer. But, would it be difference between types of LH effect? METHODS: We studied longitudinally 43 initiated cycles between August 2011 and December 2012. The stimulation aimed at comparing rFSH +rLH (18 cases), uHMG (uLH/hCG) -15 cases and uHMG + rFSH (9 cases).We compared the groups’ background, duration of stimulation, total amount of gonadotropins and LH, n of follicles>16mm, P4 and LH at the hCG day, MII oocytes, embryos transferred, and pregnancy rate. RESULTS: There were no differences in age, total stimulation period, P4 and LH levels on hCG-day, and the LH effect. 03 cycles were cancelled out of 18 in group rLH, 01 out of 16 in group uHMG, and none in the rFSH+uHMG group (9 cycles). MII oocytes, fertilization rates and good quality embryos were statistically not different. rLH group had 01 failed oocyte aspiration, uHMG group had 4 failures plus 2 ruptured oocytes and rFSH+uHMG group had 01 failed aspiration and 01 GV. Significant differences were seen to the total amount of FSH required in each arm. no differences occurred in relation to pregnancy rates per transfer. CONCLUSION: We found no evidence of different LH effects on the outcomes of mild stimulation protocol on an antagonist cycle with aromatase inhibitors plus gonadotropins. Only the amount of gonadotropins was significantly lower in the upHMG group. (p<0,01).
