Abstract
Presence of hyaluronate (HA) in cumulus oocyte and the relationship in cell maturation, DnA integrity, maturity of chromatin, sperm function, sperm-fertilizing ability. Events in spermatogenesis and cytoplasmic extrusion during the changes in the plasma membrane favors the binding of the sperm in the zona pellucida. Sperm-hyaluronan binding assay(HBA) selection most mature sperm, altered sperm swim freely finally sperm bound to HA fractions are highly enriched normal sperm(Kruger criteria).
OBJECTIVE: evaluate the role of HA (Sper-mSlow) when performing ICSI, its influence on embryo quality, fertilization, pregnancy, implantation, most important abortion rate and reduce embryos transferred.
METHODS: We performed a retrospective study period 2010 to 2012, analyzing evolution of MII oocytes using hyaluronate(SpermSlow, MediCult) to sperm selection for ICSI. We studied 581 infertile patients of which 3540 MII oocytes obtained were injected using SpermSlow and selecting those who human sperm reacted to the hyaluronate.
RESULTS AND CONCLUSIONS: Fertilization rate was 77.4% and 97.0% embryo cleavage rate, the average age was 35.68. A higher frequency of type A-B embryos 59.5% -26.5% respectively and fair to poor quality type C-D 6.6% -7.3%, was lower frequency (criteria-ASEBIR). High pregnancy rate 48.2% with 57.2% implantation rate and 7.5% lower abortion rate and embryos transferred 2.09, was significant reduction. The optimized HA-bound sperm improve and embryo quality and reduce the number of miscarriages after IVF pregnancy rates and decrease abortions. Minimize the number optimal embryos transferred. This raises further study comparing IMSI and HA, evaluating cost-benefit effect and reproducibility of the results in the same institution.
