P-66. Dydrogesterone for luteal phase support in assisted reproduction: a systematic review

Objective: To systematically revise evidence on the efficacy, safety, and acceptability of oral dydrogesterone versus vaginal micronized progesterone for luteal phase support in assisted reproductive technology patients.
Methods: The search was conducted in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) Statement. Twenty-three randomized controlled trials, observational studies, and meta-analyses were included, whose outcomes included clinical and ongoing pregnancy rates, live birth rates, adverse effects, and patient satisfaction. Data were extracted and synthesized using the Synthesis Without Meta-analysis guidelines. The quality of the selected articles was assessed according to the 2009 Oxford Centre for Evidence-Based Medicine.
Results: Most studies demonstrated that oral dydrogesterone has similar clinical efficacy to micronized vaginal progesterone, with comparable results in pregnancy rates and obstetric outcomes. Furthermore, several studies reported greater acceptance of the oral route, with fewer local adverse effects and better tolerability.
Conclusion: Oral dydrogesterone is an effective and safe alternative for luteal phase support in assisted reproductive technology cycles, especially for women who prioritize comfort and treatment adherence.
