Objective: to determine if follicular response to exogenous gonadotropin (FORT) is related to serum LH at the beginning of ovarian stimulation using only recombinant FSH with GnRH antagonist cycle.
Methods: A Prospective cohort study was performed, including 92 consecutive patients undergoing their first IVF. All patients presented with both ovaries, hormonal or autoimmune diseases and endometriosis were excluded. We also excluded severe masculine aetiology. We described all demographic and baseline reproductive data. Moreover, on the first day of ovarian stimulation, serum LH was collected. All patients were stimulated using an antagonist cycle with recombinant FSH. The primary outcome was the link between ovarian response (measured by FORT) and the baseline serum LH levels.
Results: The baseline characteristics were (mean±standard deviation) for age 35±4, AFC 11±5, AMH 2.5±1.8, mature oocytes 8±6, FORT 49% (0-100%) and serum LH Day-1 was 6.5±2.8. Correlation between day-1 LH and all included variables (age, AMH, progesterone and estradiol at hCG day) was not significant (p<5%). Moreover, Day-1 serum LH was not associated with FORT (Spearman's rho 0.090, p=0.416).
Conclusion: FORT is an essential marker of ovarian response to exogenous gonadotropin. Furthermore, the discussion about the role of LH during controlled ovarian stimulation is always in debate and endless. This research demonstrated that in patients without any previous preparation for IVF, the baseline serum Day-1 LH was not linked to a follicular response to exogenous gonadotropin.
