P-123. Identifying potential metabolites biomarkers of diminished ovarian reserve among women with unexplained infertility: Pilot study using urinary samples

Objective: Introduction: Diminished ovarian reserve (DOR) is a prognostic factor for success in assisted reproduction treatments, and among infertile women, reduced ovarian reserve has an estimated prevalence of 10 to 20%. The search for biomarkers that may be associated with low ovarian reserve and ovarian aging itself may be useful in future preventive managements and population risk assessments. A study evaluated oocyte metabolomics analyzing the follicular fluid of infertile women with DOR undergoing IVF, finding pyruvate, lactate, glutamine, glucose-6-phosphate, sorbitol, mannitol, aspartate, ornithine, and arginine. There are no previous reports with urinary metabolomic analysis of infertile women with DOR.
Objective: To assess the urinary metabolomic profile of unexplained infertile women and ovarian reserve condition.
Methods: Methods: Single spot Urine was collected from women of reproductive age between 19 and 45 years, cryopreserved at -20°C until prepared for analysis, and subsequently aliquoted for processing metabolomics analysis via 1H Nuclear Magnetic Resonance (1HNMR). Spectra were acquired on a Bruker AVANCE NeO 500MHz spectrometer. Data processing a and metabolite identification were performed using TOPSPIN and Chemomx respectively. Multivariate statistical analysis was performed in MetaboAnalyst 6.0. The urine samples from women with UI were divided in two groups without and with DOR (AMH < 1.2 ng/ml, by Elecsys® AMH assay Roche Diagnosis).
Results: Results: Thirty-four metabolites were identified and quantified in the urine samples. PLS-DA analysis revealed possible distinctions of the metabolic profiles of the DOR samples vs. non-DOR group. Twelve metabolites (VIP >1) seem to be responsible for the observed separation in the PLS-DA plot, including o-phosphocholine, sarcosine, dimethylamine, creatine, formate, glycolate, citrate, urea, succinate, trigoneline, hypoxanthine, and acetone. Trigoneline, alanine, phenylalanine, citrate, 3-hydroxysobutyrate, lactate, dimethylamine, 3-4 dyhydroxymandelate, 3-aminoisobutyrate, 3-hydroxyisovalerate, choline, o-phosphocholine, sarcosine, hypoxanthine, valine showed higher concentrations in the DOR group, whereas hippurate, acetate, malonate, gallate, histidine, trimethylamine N-oxide, sucrose, succinate, creatine, urea, glycine, methylamine showed higher concentrations in the normal ovarian reserve group.
Conclusion: Conclusion: This pilot study identified, in urine samples, potential biomarkers of diminished ovarian reserve among women with unexplained infertility. This finding may contribute to the refine assessment of this condition, serving as a basis for future research.
