P-151. Liraglutide in Assisted Reproduction: Current Evidence and Clinical Guidance

Objective: To review the scientific evidence on the use of liraglutide in women with obesity and/or PCOS in the context of assisted reproduction, and to provide clinical practice recommendations.
Methods: A literature review was conducted in PubMed, Cochrane, and Embase, including publications from 2015 to 2024 in English and Portuguese, regarding the use of liraglutide in assisted reproductive technologies. Six articles were identified, of which two were excluded for not meeting the inclusion criteria.
Results: Liraglutide (1.2–1.8 mg/day), alone or in combination with metformin, showed increased spontaneous ovulatory cycles in women with PCOS and reduced total gonadotropin doses in IVF cycles in some studies. However, there is no robust evidence of consistent improvement in live birth rates. Use during active attempts to conceive is contraindicated due to the lack of fetal safety data. Guidelines recommend effective contraception during treatment and discontinuation of the drug at least four weeks prior to timed intercourse, intrauterine insemination, or IVF.
Conclusion: Liraglutide is a promising tool in the preconception phase for patients with obesity and/or PCOS, providing metabolic improvement that may positively influence reproductive outcomes. Its use should be limited to the period of clinical optimization, with discontinuation prior to conception according to international safety recommendations. Additional studies are needed to confirm its direct impact on live birth rates.
