Objective: To determine the optimal protocol for incubating thawed single blastocysts in hyaluronan-enriched transfer medium (HETM) to improve pregnancy rates in assisted reproduction.
Methods: We performed a prospective cohort study including 1310 patients undergoing frozen embryo transfer (January 2023- February 2025). Patients were divided into two groups: those receiving HETM and those receiving standard transfer medium. Two incubation protocols were compared: Protocol 1 involved incubating embryos in the transfer medium (HETM or control) for 2-4 hours after thawing. Protocol 2 involved an initial incubation in standard medium, followed by exposure to HETM (or control) only 15-30 minutes before transfer. Pregnancy rates were analyzed using chi-square tests; p<0.05 was considered significant.
Results: In Protocol 1 (n=1088), pregnancy rates were similar between HETM (33%) and control (35%) groups (p=0.572). However, in Protocol 2 (n=222), a significantly higher pregnancy rate was observed in the HETM group (56%) compared to control (37.8%) (p=0.0216).
Conclusion: Prolonged exposure to HETM (2-4 hours) did not improve pregnancy outcomes. However, short-term exposure (15-30 minutes before transfer) significantly increased pregnancy rates, suggesting that brief incubation with HETM may enhance embryo implantation in frozen single blastocyst transfers.
