P-041. The Use of a Sperm Separation Device Selects Spermatozoa with Better Morphology and DNA Integrity

Objective: To evaluate the application of a sperm separation device on seminal parameters and sperm DNA fragmentation index.
Methods: Seminal samples were collected from 27 patients at the IMR-CRP who attended a fertility consultation between June 2024 and March 2025. Only samples with a volume more than 1.5 mL were considered. Seminal quality was evaluated before and after sperm separation using the sperm sorting device (Ca0, Lenshooke), including sperm morphology, concentration of progressively motile sperm/mL, and sperm DNA fragmentation index (DFI). Standard procedures for seminal sample processing with Ca0 were followed, using 1 mL of sample, 0.9 mL of washing medium, and post-incubation sample recovery (30 minutes). Normality of the evaluated parameters was assessed, and depending on the results, the Shapiro test was applied for normally distributed samples, and the Wilcoxon signed-rank test was used for those without anormal distribution.
Results: The Ca0 device selects spermatozoa within the microenvironment created by the device's membrane, enabling the replication of the procedure by various professionals. High values of motility and morphology were found, along with a significant increase in progressive motility per mL (p=0.001), morphology (p=0.001), and a reduction in sperm DNA fragmentation index (p=0.001) compared to baseline values. The existence of a correlation was analyzed using the Spearman's rank correlation coefficient (p-Spearman) between the parameters, revealing an inverse relationship between motility per mL and DFI (-0.337), morphology and DFI (-0.344) in native samples, as well as morphology and DFI in post-selection samples with Ca0 (-0.311).
Conclusion: The Ca0 device provides an efficient and non-invasive alternative for sperm selection, offering a standardized procedure that significantly reduces DNA fragmentation index and improves sperm morphology. It also shows a negative correlation between motility and morphology with DNA fragmentation index.
