Objective: Evaluate differences in blastocyst morphometric parameters-inner cell mass (ICM) area, blastocyst expansion (area excluding the zona pellucida), and estimated trophectoderm cell count (ECC)-between embryos of different chromosomal status (euploid, aneuploid, mosaic).
Methods: This retrospective cohort study included 62 human blastocysts from assisted reproductive treatments with PGT-A between January 2024 and March 2025. Embryos with poor image quality, imprecise measurements, or inconclusive biopsies were excluded (n=22). Embryos were individually cultured in G-TL medium (Vitrolife®) under oil at 37°C with 6% CO₂ and 5% O₂. Development was monitored at Day 5 (120-122h) and Day 6 (144h). Images were taken before biopsy for morphometric analysis using Motic Images Plus 3.0ML®. ECC was estimated based on cell size. Embryos were categorized as euploid (n=32), aneuploid (n=22), or mosaic (n=8).Spearman’s rank correlation assessed associations using binary grouping (euploid vs. altered). Pairwise comparisons were performed using Mann-Whitney U tests. Significance was p < 0.05.
Results: Spearman’s correlation showed a trend between euploidy and ECC (ρ=0.233, p=0.058). Similar trends were seen for ICM area (ρ=0.200, p=0.104) and blastocyst area (ρ=0.146, p=0.239). Mann-Whitney tests yielded: blastocyst area (p=0.503), ICM area (p=0.287), ECC (p=0.106). Significant ECC differences were found between mosaic vs. euploid (p=0.003) and mosaic vs. aneuploid (p=0.007).
Conclusion: Findings suggest ECC may be a non-invasive indicator of chromosomal constitution. Mosaic embryos appear to follow distinct proliferation patterns. Larger studies are needed to confirm these trends and improve embryo selection strategies.
