P-048. Can the hyaluronic acid based injection medium and the Z-potential technique increase the pregnancy rate?

Objective: Evaluate if the pregnancy rate can increase in non-normozoospermic patients using the hyaluronic acid based injection medium and the Z- potential.
Methods: This ongoing prospective randomized study including 35 couples who underwent on high-complexity egg donation procedures between 2023-2025. A total of 500 mature oocytes were fertilized via intracytoplasmic sperm injection (ICSI), with an average of 14 oocytes per couple. Participants were randomized into four groups: Group 1 (Gradient + PVP), Group 2 (Gradient + PVP + Z-Potential), Group 3 (Gradient + Sperm Catch™), and Group 4 (Gradient + Sperm Catch™ + Z-Potential). Inclusion criteria included at least one altered semen parameter according to WHO guidelines (2021) and at least one good-quality embryo obtained. A total of 43 embryo transfers (fresh or cryopreserved) performed until February 2025 were analyzed.
Results: No significant differences in blastulation rates were observed among the groups (p<0.05). However, Group 4 showed significantly higher pregnancy rates compared to the others (88.89% vs. 33.33-42.86%; p<0.05). No significant difference was found between Groups 1 and 3.
Conclusion: Combining 3 sperm selection techniques with different principles significantly improves pregnancy rates. Additionally, the hyaluronic acid-based injection medium appears to be a superior, non-toxic alternative to PVP due to its better biocompatibility with the oocyte.
