Objective: To describe our experience in managing severe Ovarian Hyperstimulation Syndrome (OHSS) with outpatient transvaginal paracentesis Methods: From April 2007 to July 2009, in 10 hyperstimulation cycles, transvaginal paracentesis was performed. The data was retrospectively analyzes Results: The median of paracentesis performed by patient was 2 (1-2,25), considering that one patient conduct 7 paracentesis. The average volume of ascites withdrawn per patient was 1807±805ml. Being the patients hemodynamically stable, there was no limit on amount of liquid retrieved or in the number of procedures. There were no complications related to the procedure and none of the patients required hospitalization. Conclusion: The outpatient transvaginal paracentesis is an effective and secure measure in case of ascites, accompanied or no by hydrothorax, in management of severe OHSS
