Use of ovarian reserve markers and variables of response to gonadotropic stimulus as predictors of embryo implantation in ICSI cycles

Abstract

Objective: To evaluate the potential to predict embryo implantation from endocrine markers of follicular reserve and variables of ovarian response to exogenous gonadotropic stimulus in cycles of intracytoplasmic sperm injection (ICSI). Methods: We prospectively evaluated 45 ICSI cycles in women with 38 years of age or less, regular menstrual cycles, with no endocrine diseases or endometriosis, with male factor of infertility, with both ovaries and no recent hormonal treatment. Ovarian reserve was assessed by basal measurements of estradiol, folliclestimulating hormone and anti-müllerian hormone, and ovarian response to stimulus was assessed by dominant follicles count in the day of hCG administration, total number of oocytes retrived from follicular aspiration and the number of morphologically mature oocytes retrieved. Receiver Operating Characteristic curves (ROC – AUC) were obtained to determine discriminatory capacity of each variable to identify embruo implantation. Results: None of ovarian reserve markers presented significant capacity to predict embryo implantation. Among the variables of ovarian response, we observed significant capacity of dominant follicles in the day of hCG administration (ROC-AUC = 0,748; p < 0,01) and of the total number of oocytes retrieved (ROC-AUC = 0,665; p < 0,05) to assess embryo implantation. Conclusions: The evaluation of ovarian response by dominant follicles count in the day of hCG the total number of oocytes retrieved seems to be of good perspective in regard to prediction of embryo implantation in women submitted to ICSI.
