Our Experience With HMG For Ovarian Stimulation Following GnRH Agonist Supression

Abstract

During recent years, it has been much debated whether suppression of LH levels during ovarian stimulation is beneficial, harmless, or harmful in relation to the ART. These discrepancies reflect that the exact requirements of LH for normal follicular development and oocyte maturation are not known. Trial have shown that severe suppression of midfollicular LH levels in the serum occurs in women treated with rFSH, resulting in significantly depressed levels of circulating E2 as well as in a poorer outcome of ART as compared with women treated with hMG. The aim of the present study was compare the number of mature oocytes observed that women treated with GnRH agonist followed rFSH with addition or not the hMG. 89 cycles was included and separated in two groups: group I-cycles stimulated with GnRH agonist+rFSH+hMG and group II–cycles with GnRH agonist + rFSH. The analogues was initiated at 22° day of the cycle and maintained until the blockade. During the first 5 days of ovarian stimulation, all the patients were treated with rFSH (300IU). After, the group I’s patients were treated with hMG (300IU) and group II’s patients were treated with rFSH (300IU). When two follicles with 16 – 17mm were observed, hCG was administrated. The pickup was realized 34 hours after hCG. There was no significant difference in the numbers of mature oocytes picked-up and number of embryos to transfer. It was concluded that hMG additioned with ovarian stimulation’s long protocol offered adequate number of mature oocytes and normal embryos to transfer.
