Abstract
A case of ovarian ectopic pregnancy after treatment with in vitro for primary tubal infertility is described. The patient was a 39 year old woman with left blocked and right absent tube. After ovulation induction 17 oocytes were retrieved and 3 days later 5 embryos were transferred to the uterus. Serum hCG was positive 12 days after the transfer. After a significant increase in hCG leveis vaginal ultrasound was performed when we found an irregular image within the endometrial cavity and enlarged right ovary with an irregular mass. As the patient refused to perform a laparoscopy, we started monitoring hCG leveIs, that started to decrease until 4500mUI/ml, when the couple moved back to their home town. One week later the patient developed a severe pelvic pain and a laparotomy was performed. The finding was ovarian ectopic pregnancy that was successfully removed, keeping the ovary intact. We discuss the mechanisms that might have determined this ovarian pregnancy and the possible treatments.
